Returning to Sewanee After a Leave
Loading...
For full consideration, applications for reinstatement must be received by October 25 for Easter semester and April 1 for the Advent semester; this allows students who are approved for reinstatement to register for classes with their cohort. Applications received after these dates will be reviewed on a rolling basis, with a typical cycle taking at least two weeks. No applications for reinstatement will be considered within ten working days of the start of the next full semester.
If your return to Sewanee is contingent upon receiving financial aid, please be sure you have submitted appropriate information to the Financial Aid office BEFORE completing this application! See
http://admission.sewanee.edu/financial-aid-details/
for information.
If you attended another college or university, you will need to upload the "Statement of good standing" at the appropriate place below. Have this document ready as a PDF before you begin. You'll also attach a screenshot or scan of your schedule (with grades, if available)--though this does not replace the official transcript you must send to our Registrar.
If you worked while away, and you are required to document it (or just want to prove evidence of how you spent your time), create a single PDF showing your first and last pay checks. You should black out your social security number. Have this document ready as a PDF before you begin.
If you did volunteer work, you should prepare documentation in a similar fashion (a single PDF with a letter or other documentation detailing the dates you worked and approximate hours per week).
With any PDF uploaded to your application, be sure your name is clearly part of the file name.
A student with an outstanding balance on his or her student account will have his or her application placed "on hold" until the account is paid in full.

Sewanee has two parameters for students receiving aid through the University: (a) they must meet minimum academic progress standards, and (b) aid is offered for only 8 semesters of enrollment, whether completed or not. If you have been placed on academic or financial aid suspension, and/or if you have already received aid for 8 semesters, you will need to appeal to have your financial aid restored. In the space below, describe why you were unable to meet progress standards and/or finish within 8 semesters, and what has changed that will allow you to be academically successful, with a plan for success.
Medical Documentation
The process for applying for reinstatement requires that you download the document "Medical Documentation for Reinstatement" found at
http://www.sewanee.edu/media/dean-of-the-college/docs/Medical-Documentation-for-Reinstatement.pdf
. (You may need to copy-paste the URL.) Your health care provider will have to complete the form with you!
That form and any related paperwork must be submitted electronically--if not attached to this form (see below), then as scanned files (PDFs) to the Office of Student Success at
studentsuccess@sewanee.edu
. All documents should be clearly labeled. No application will be considered by the committee on reinstatement until it is complete.
Please explain in brief the steps you have taken to address the health concern that prompted your leave of absence, and please outline your plan for maintaining your health if you return.
Please attach (a) the form Documentation for returning from a medical withdrawal, (b) the signed releases allowing Wellness Center staff to communicate with your health care provider, and (c) any relevant additional documentation from your health care provider. All are at
http://www.sewanee.edu/media/dean-of-the-college/docs/Medical-Documentation-for-Reinstatement.pdf
. Please be sure documents are clearly labeled with your name.
Note: If you are not attaching documents above, please tell us how we will receive those documents and when we should expect them.
Personal Statement
In your own words, why did you have to leave Sewanee? (Or why did you choose to, if it was your choice)?
If you were suspended: Please explain to the Committee (a) how you have taken appropriate responsibility for your actions which led to your suspension, noting specific points of progress, and (b) how you will return with the dedication and maturity for improved performance, with specific plans in place to make sure that those problems do not affect you again.
If you attended another college or university while away, please complete the information below. If not, skip to the next section.
Institution attended: List (a) institution, (b) semester or term attended, (c) courses taken, noting credit hours of each, and (d) when classes will (or did) end. (If classes have ended, send an official transcript to our Registrar.)
Upload a single PDF with your "statement of good standing" from the other college as well as your course schedule or final grades (like a screen shot of your online record). This does NOT replace the official transcript--for Committee use only! Be sure the file has your name in the title!
